It is a platform for chronic disease case managers. Keeping track of body indices is vital for chronic patients. Thus, this platform would offer both patients and case managers a convenient and efficient way to communicate. In this way, we could make sure the conditions of patients remain stabilized while also reducing the cost of healthcare.

What I did

in the project

I was in charge of constructing a platform for case managers. It must include some key functions, such as clearly showing the indices of all patients, easily filtering data to compare different cases, and communicating the information to patients.

By organizing all of the data into a table, the managers could conveniently view the body indices of all his/her patients. The pastel colors would be easy on the eyes so the user could focus on viewing their cases.

There would be a considerable amount of data, and if the filter still took up space, then the screen would be relatively crowded. Hence, I set up a collapsible filter on the upper left side. With these filters, managers could easily access their patients’ data. They could select time periods and enter the name of the patient, and the website would show all of the patient’s data during that time period. It is a convenient and efficient means for managers to view the data and track the condition of their patients.

By clicking the name of the patient, a window would pop up that shows data as charts. The charts could help managers observe the fluctuation of the data and also set a standard value for patients.

On the top of the pop-up windows, there would be a button for a chat box. The chat box would allow case managers to communicate with patients easily and give them some guidelines to follow. It could help patients control their condition effectively. The pop-up windows concept would be used to avoid too many web page changes. After all, fewer web page changes would make the platform more convenient and user-friendly.
